The maximum dose of either is based on potential liver toxicity as both are metabolized by the liver. If you are reaching the max dose on one, switching to the other is dangerous. New docs typically recommend alternating doses for infant fevers as a method for more effectively reducing fever. I don't recommend this as it's confusing to parents and it efficacy is questionable. In my experience, kids vary and respond better to one or the other. If one doesn't work, try the other ...stick with the one that works. In my experience, most infant fevers respond better to ibuprophen (advil, pediaprophen). Huh....we have routinely been taught that alternating the two is fine as long as they are given every 3-4 hours and dosages of either are not exceeded. So I did a quick review of pharmacology, and while they're both metabolized by the liver, the pathways are different, and it doesn't appear that the effects should be additive.... BUT, here's what I found at NIH. CONCLUSION: Acetaminophen and ibuprofen are commonly being used in an alternating manner for management of fever. There is presently no scientific evidence that this combination is safe or achieves faster antipyresis than either agent alone. There is evidence that the improper use of these agents may cause harm. Despite 29% of participants citing American Academy of Pediatrics recommendations as the basis for fever management, no such policy or recommendations exist. The observation that this practice is more common in younger practitioners may reflect their continued anxiety about fever (fever phobia). Until properly controlled studies have assessed the risk of combining these 2 products, practitioners should proceed with caution. It looks like the jury is still out on any dangers of alternating these medications. Of course I haven't looked at any original sources, though. Seems that there appears to be no benefit, and possible detriment. It looks like the biggest risk is renal rather than hepatic toxicity though.... There are better ways of hydrating a person than with gatorade....but if you've just had typical vomiting for a day, are recovering, and aren't *seriously* dehydrated, gatorade will be okay.... Michelle's giving good advice. Also...alternating acetaminophen (tylenol) and ibuprofen is a good idea if you're getting close to the maximum recommended doses of either. Some folks like to push other people's buttons. Somebody got a HUGE catharsis with this high-yield button pushing. Now you're fumin' and somewhere someone with a hot finger is feeling smug and pretending not to have known what she/he was causing.... See how ridiculous it is??? It's all for effect and probably attention. When somebody hits your sore spot, you can CHOOSE how to respond. This came out badly for you. Jen clearly stated we're talking about drugs that are unnecessary. OIC .... I hope that we're getting better about that. It is hard not to cave to patient pressure, and from what I see in teaching hospitals, we ARE doing better. I imagine that in the private world the change is slower. I did take my child in to have his infected ear looked at. IMO, that's a really weak justification. If you feel you have to give them something, why not give them mega Vit C bills or powdered hydrogen dioxide? Why is it a weak justification? There are some medications that can help with cold symptoms. Why, if a patient wants something to help with cold symptoms, is it a problem for a doctor to prescribe something to help with cold symptoms? In my experience, MANY doctors will discuss with patients non-prescription supplements that can help keep them healthy.... Common ones that I KNOW are often recommended are Vit C, Vit D, Calcium supplements, Vit E, Black Cohosh, CoEnzyme Q10, soy, Omega 3 Fatty Acids. There are more. There are side effects of many medications. Those should be discussed with patients. Most people are capable, in my opinion, of deciding with their physicians whether the risks/side effects outweigh the benefits, and if any given medications are something they desire to take. Physicians prescribe the best medications we have to help patients recover and feel better. Nobody's gonna force them on you. If you aren't satisfied with what those options are, then you're usually not forced to follow any physician's recommendations. But at least the medications you're being prescribed have been tested heavily. There sometimes are still problems, but a LOT more is known about effects of prescription medications than is known about most herbal supplements. -- A conservative is just a liberal who's been mugged. One thing that I find to be extremely disrespectful is for someone to tell a person that he/she *loves* that something in their behavior won't be tolerated. That's what you tell a child, not a spouse. To have a need to control a person to that extent is a much, much bigger problem than a woman telling a friend that she loves him. It's extremely degrading. What is acceptable/unacceptable in a relationship is something that should be discussed between the two of you, not decided by dz.com. I cannot even begin to fathom what "politically correct math" could possibly be. I agree that since many, many parents do not take the time to teach their children how to get along with others in the world, the schools have inherited a big part of that job. Teaching children to appreciate people of diverse backgrounds is a good thing.
