  5. Shouldn't they ask and let the option be decided on by the patient, or should the patient automatically know that the test. Yes and no. First let me explain a little about this test. The CA 125 is neither sensitive NOR specific for ovarian cancer. It's sometimes called the "ovarian cancer test" or the "endometriosis test" but it's really both and neither. The CA 125 is a marker for peritoneal irritation. The peritoneum is the lining that lies over the abdominal organs (like saran wrap that is just covering everything in there and letting it all move and slide) When that lining gets annoyed, it releases that protein. Many things can cause that number to bump: benign causes include normal menstruation (your period), endometriosis, uterine fibroids (benign tumors), pelvic inflammatory disease (infections), pancreatitis, and liver disease; malignant (cancer) causes are not just ovarian but also cancer of the uterus (endometrium), pancreas, intestines, lung, even breast. So a positive test can really just ask a hell of a lot more questions than it would answer in a woman with no symptoms. That's why it's NOT a good screening test. And... there's the whole insurance and cost effective thing. We as doctors are often judged by our ability to predict the future. If it ends up being cancer... no one wants to wait until there's symptoms for cancer. Early detection is the expectation. BUT.... if it isn't and now you have this "cheap" test (even at $300) that is elevated... now it gets expensive. So now what? Schedule an ultrasound? Waht if the ultrasound is normal? Order a CT? what if the CT looks normal.... (add cost of CT and risk of radiation) ... do you now do a diagnostic laparoscopy (surgery to look in the abdomen)? (add on cost of surgical procedure and risk of infection/bleeding/injury to adjacent organs) Dx L/S looks normal... do you now just remove the ovaries even if they look normal? (risks of menopausal symptoms).... pathology comes back normal (no cancer).... Now.... you've done a CA125 Pelvic ultrasound CT of the abdomen and pelvis with/without contrast Diagnostic laparoscopy Bilateral oopherectomy all for normal ovaries all at what cost and risk? In many ways, I do give the patients choices.... but I also try to temper the counseling with individual risk factors.... something that those internet chain mails don't do. If you have questions about tests like this, do ask your doctor.... but LISTEN to what he/she says. Don't just go in there thinking "I am entitled to have this test done." and definitely don't think that medicine is just that easy that we look at one blood test and will save every one.

Phallos, expert witness testimony explained proper dissection technique: Incise the skin of the penis by making a vertical incision on the dorsal surface of the penis from the pubic symphysis to the glans penis. At the base of the body of the penis, identify the fundiform ligament of the penis. Deep to the fundiform ligament, identify the suspensory ligament of the penis. Continue to dissect the dorsal aspect of the penis. The deep dorsal vein of the penis is located in the midline. In this case, there was legal precedent set that the length of that deep dorsal vein is the "length" of the penis.

  8. Hiya Doc K...how the heck are ya? I've been BUSY!!! And "real life" drama has occupied too much of my time. I took this week to escape to the Wild West to get back to nature. If you haven't been to Yellowstone - GO! It was FANTASTIC! Not just Old Faithful but the springs and the animals and the nature and.... Yellowstone is AMAZING. (but the Black Hills are still my favorite)

  11. I just made a simple comment about the "change" that we see in politics. THEN came the demeaning and insulting comments from a mod about being silly. I further defended my statement that the President SHOULD set a good example.... is that silly? I remember others venting on here about other prior Presidents not being Presidential... and gee.... you agreed with them. (I did a quick search of your posts revealing 315 with "Bush" one here and here... but I didn't find a SINGLE ONE where I called you silly or stated that your opinion was "unworthy of " blah blah blah) But since I don't agree with your opinion of the current administration... then I'm silly and just "hatin" From some of the interactions that I have witnessed on this forum, I'm glad that I haven't had to work with many here on a personal basis. There's a basic respect for others that seem to be missing. I might not agree with others. But I understand that they are entitled to their opinion. EVEN IF IT IS DIFFERENT. My opinion IS and HAS been for several YEARS that politics are corrupt. It hasn't changed with this "new" change of Presidents. If you believe in this hope and change.... WONDERFUL for you. But my opinion is different. California is wrong with doing this. President Obama is not leading in a way that I would want a President to lead. You can have a different opinion.

When he begins on a platform of "Change"... and there is none. Promises "transparency" but then tries to push through legislation without an allowance of time for it to be read.... HE'S THE PRESIDENT. He should act like it. But... it's an administration of "change"

  14. I'm going to combine a couple of the topics above. Hate/prejudice and apathy I think the biggest threat would be local extremist that are willing to take any measure to be heard coupled with a population that just seems content, almost entertained, with watching these horrific events on the television. The extremist don't have to be just religious. They could be nationalistic, animal rights activist, racist, political. According to this wiki site (I didn't want to click on any .mil or .gov sites about extremist groups.... so deal with wiki ) there are at least 33 organizations in the US that are considered extremist groups. (and for those non-US readers, yup, you gots em too) So my answer is more a reflection of us as a PEOPLE... not a blame on some outside influence. We are the biggest threat. But... I can live in hope that we will wake up and see what we are doing. Someday, we will understand what Plato's Cave is... and how just watching life on a screen... isn't life.

  16. I have to kind of agree with this rant. I've never had to work food service.... thankfully! Because some of those customers can be complete unappreciative asses. It would be different if the person coming in at least acknowledged that he/she is keeping the staff there working an extra 30-60minutes just for them, If the tip was reflective of that sacrifice, If there was even a "thanks." But often, as heard in this thread, it's all about what they feel "owed." The sign says til 4am... "it's only 3:55 - feed me, bitch." Yeah, that goes over well. Enjoy the iced tea. Yes, management is important, but how many restaurant owners are there at 3:30am?! So the junior mgt is there with "orders" to keep the doors open... and then people take advantage of that. So... I think it's as much the fault of the people taking advantage of it as it is those in charge not allowing them to take advantage. Just be better, nicer, and don't forget to tip the waitress.
  17. Using your above anecdotal examples: Military medicine (controlled by government) Will take 2mos and a week. Civilian side of the house. 2 weeks. Which do you think is better? (and ... the wait time on an ultrasound?! I would need a bit more information on that. What I SUSPECT that story is about is that it is possibly a follow up ultrasound to be done in the future. I HIGHLY doubt that it would take that long to get an ultrasound. In my clinic, I have a machine down the hall, or we have two facilities that we can refer to OR the ER is right in the attached hospital. I would need more information on the case that you mentioned... but I think the bullshit flag was rightfully thrown)

    Unless, of course, there are complications. Risks include anesthetic complications (including death), infection, injury to adjacent organs (bowel, bladder, blood vessels) fistulas tract formation (vesico-vaginal fistula, recto-vaginal fistula), scar tissue formation, post operative pain/dyspareunia (pain with intercourse) Not a guaranteed win-win. Surgery should not be viewed lightly.
