  4. Well now that you mention it I chopped the main of the Sunday that did not deploy last Monday. And no it was not the same pack job. new lines on the set 400 tension knots with 255 in the saddle up front.. Tried to clear it but the spin was hard and G's were incredible. I watched the video of the deployment and it looks like the slider stops and stabilizer line attachments are coming down with the slider pushing the lines down with it. once the stabilizer releases and goes up the lines are flopping around below the grommets and wrapped them selves into knots. By the way I did not pack these malfunctions myself and could use any advice toward helping my packer with Tension knots on set 400 and 366 I also have 1 precision 365 that likes to beat me up on opening.   6. Yes I had this mal on a Vector 2 when a master rigger assembled and packed a new main for me while I was working. the toggle was stowed past the stiffener and was not removable during flight. My rig was jumped by another TI on the first jump and had an uneventful Reserve ride until he landed and the rear line group fell off the riser into his hand but that is another story all together...   8. that was the loop with the most fraying on the top and I spread the loop out when I pulled on it to inspect it. (Notice the bunching at the grommet)That pushed the outside down about .25 of an inch. it was single thru the cutter when closed. We also leave the flap open and the riser up for inspection. Moving to fast and not looking for the mistakes is the biggest danger to every skydiver but especially when doing Tandems. Thanks for your insight.   9. You are so right and that is what disappoints me the most. My habit has always been look touch feel when doing my pre flight. I even touched both loop ends on the other side but never even turned the riser completely over just a slight glancing look that wasn't even a look at all.   11. Second ring only... as I look back I checked the cable side to insure 1 and 1 thru each loop but never looked at the front side....I could not budge either drogue release.   12. Drogue 3 ring double loop was thru the second ring. I stood on both the primary and secondary release handles and could not extract either... Yes I missed it in my pre flight...... I pulled the drogue 3 ring up and called myself looking at it as I do before putting every rig on but I was complacent like looking at your watch then realizing you did not even read the time..... Tension knots on right side wont clear fly's straight but folds under and dives when flared. fly to runway cutaway at 3000 nice landing main landing area everything retrieved in 30 minutes. Second.. Today... Drogue in toe.... pull main drogue release at 4500 wont extract secondary at 4000 wont extract pull reserve at 3500 wont extract... regroup!!!!!! look oh SHIT Cable housing not reserve handle FUCKING idiot... Reserve handle pulled and cleared 2500..... POP.... Cypres fire saddled out under reserve between 1600 and 1450. ..... land off main runway 100 yards... See pics of complete cutaway sequence from first and both uncut reserve closing loops of second... Both closing loops are frayed on the top edge slightly possibly from the cutters??? Both reserve loops were installed new by me 2 days prior when assembling tandem rig for service. I always said I would stop jumping if cypres saved my life...I sure think this was too close.... I dont like saying die or death but have no problem with jokes like " you will need to stay in the arched position for the entire freefall portion of your sky dive....50 seconds if the parachute works 75 seconds if it does not" or....... " if it looks like we are going to hit the ground at an xecelerated speed take a deep breath and hole it.......make them wait a few seconds or ask why then reply .... There is video of the complete event for documenting the record I dont have any to post at the moment. 5.5k 10.38 seconds or something like that was the average. they took an hour after 104 to clean up and get purdy for the cameras on 105. 2 x 182's and 1 x 206 who,s door quit closing making the record even more incredible because Luther and Angela just kept climbing to 5.5k with the door open and it was cold as heck that morning. there was no military team to assist nor was there any huge contingent of volunteers. The rigs were packed by only 2 packers moved to and from the landing and loading area by 1 of 5 people and there 3 persons in the media who jumped in and assisted threw the night. Every rig got 2 inspections before being cleared for its next use and the passenger harness was monitored and adjusted after every landing. Luther Kurtz and Angela Bishop knew once they started the record was their's and shortly there after so did everyone else.   16. Luther Kurtz and his sister Angela Bishop made 105 tandems in less than 24 hours tuesday and wednesday at Skydive Harbor Springs in MI. Congratulations to them both on a great job. With a small team and little endorsements the event was a huge success. There were no malfunctions or injuries and all jumps were made on 4 rigs 3 Sigma Tandem "Rigs with 370 sigma Canopys and 1 sigma that was used 5 times with precision 365.   17. What I see when TI's put helmets of any kind on their passengers is passengers hanging lower in the harness which causes its own problems. When I wear a frap and my student wears a frap communication is harder and visibility is less so i only wear them on really cold jumps. I can only imigine the impact force of a hard opening with 2 hard helmets would be brain rattling. I put my head next to the passengers during opening which is hard to do with a hard helmet. Never Track behind the TI you need to be in the TI's sight at all times after exit. the n TI needs to watch you Track and have you in their sight as they deploy!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!! It is the TI's Decision but the TI should never disregard the video persons safety and the same goes for allowing unsafe video person to shoot tandems......   19. The numbers say Tandem is safe. the numbers do not address training, equipment or the complexity of tandem in general. As we all see from past tandem fatality's when something goes wrong it can turn tragic in short order. It is the training that will help the TI assess the situation and produced a positive outcome. Having those skills is required for becoming a TI. Also the numbers are skewed dramatically by sport fatality's under a fully functioning parachute. It would be very interesting to see how the numbers crunch with those removed for a realistic view of malfunction fatality's. My bottom line is Tandem is just as dangerous and demands better response skills when something goes wrong because of the complexity of tandem systems as a whole. I love the DZO's who want to pay the person performing tandems $30 which is minimum pay as well as put on a handycam for $20 a jump. You want me to save you a slot that you fill for $25 extra and pay me less then outside video while you charge the customer the same price. Greed breeds danger is all I can say.....   20. As I understand them US tandem regulations apply to students and the use of tandem for training purposes. The use of tandem rigs by 2 rated skydivers is only limited by their creativity and stupidity. Having called for permission or waivers for (alternative tandems)tandems outside the norm I have been told more than once it is not needed for 2 rated skydivers. The first was to do an Austrailian ( passenger Female) wearing harness backward facing TI during freefall, Nakid is proffered. Another Tamden MR Bill sure makes holding on a non issue. Passenger facing forward can unhook self after opening and climb up risers to shoulders and depart at will.   21. Billy after reviewing the video many times it is hard to say what went wrong with the landing other than it appears the passenger straight armed the instructor on landing. Since I can not see the TIs arms at any time during the landing it appears they are covered by the passengers and that he had assistance from the passenger who pushed his arms straight out instead of down which crippled the instructors ability to safely land the pair.   22. I had one who was so scared he started puking right after we tightened the laterals . On our knees in a 182 facing forward at the door. Must have been 1.5 gallons of puke... before I could get the puke bag out of his harness. he caught his breath just as we got on top... I asked do you want to ride the plane down??? He looked between his legs at the steaming pile he had deposited and said "hell no I aint riding down with this shit" I gave the quickest cut, door and exit while giving the pilot one of those sorry about this BUT SEE YA looks...............Dude was fine after that... Nothing left to spew I suppose. Talk about one pissed off pilot....   24. I currently use HD HC-5 with cameye but want stills this season. I like being able to control stop start with cameye. Post your setup and why it works for you. I see people discussing the hero.
