I've screwed up twice in a big way by my standards. I deployed at about 1800 feet after losing altitude awareness during an RW jump. I may have gone lower if an up-jumper with me hadn't (almost literally) smacked me on the side of the helmet and given me a pull signal (what the hell is he pointing at . ). That was stress and sleep deprivation at work. About a year later I fought line twists to 1400 feet before I realized I hadn't checked altitude. I lucked out and cleared them at the same time.
