  4. Not about the Catapult, but the Reflex container. You should check (or have someone check) the tightness of the grommets in the container. I believe that there was at least one fatality caused by entrapment of a main suspension line on the container (snagged on a loose grommet) causing a horseshoe, which led to a main-reserve entanglement.   6. There has been some discussion in the past about the Rapid Transit System (RTS) rig, with the Reserve and Cutaway handles revesed. I was researching a fatality that occurred at DeLand in 1980, which involved an RTS. There was another fatality in DeLand shortly after that, also RTS-related. Does anyone remember how many fatalities were linked to this "reverse-handle" configuration ? The RTS was made in DeLand, so it's no surprise that there were many in use there, but does anyone know if the "reversed-handle" RTS was widely distributed ? How long and how many rigs before they returned production to the conventional configuration ? It seems that the RTS never recovered from the stigma of the accidents, and went out of production.   9. I'm trying to locate info from a 1980 fatality at DeLand FL. None of the databases seem to go back that far. Is there any way to locate something like this other than looking through the 1980 issues of Parachutist ? Kevin K.
  10. The Columbia STS-107 mission lifted off on January 16, 2003, for a 17-day science mission featuring numerous microgravity experiments. Upon reentering the atmosphere on February 1, 2003, the Columbia orbiter suffered a catastrophic failure due to a breach that occurred during launch when falling foam from the External Tank struck the Reinforced Carbon Carbon panels on the underside of the left wing. The orbiter and its seven crewmembers (Rick D. Husband, William C. McCool, David Brown, Laurel Blair Salton Clark, Michael P. Anderson, Ilan Ramon, and Kalpana Chawla) were lost approximately 15 minutes before Columbia was scheduled to touch down at Kennedy Space Center.   13. The American public has a perception that the U. S. Space Program runs on a large percentage of the federal budget. It actually uses less than 3%. The Space Shuttle program is on its way to shutting down. The last flight will occur later this year. The next manned vehicle to fly from the U. S. after that will be a minimum of 5 years away. I have a feeling that a year from now, when folks realize that the U. S. has no longer has the means to fly people in space and that we will be paying US$50M for each of our astronauts to fly to the International Space Station on Russian ships, many will think, "WTF". I think that if it were put to a referendum vote, the majority of U. S. citizens would be in favor of increasing Space Program funding and not losing our manned flight capability.   23. Here's the D-21 at the USAF Museum at Wright Patterson. They don't have an M-21, but that's a YF-12A, predessor of the SR-71, in the background.
