If I remember correctly, the only way to modify a pdf document is to have the full version of Adobe Acrobat. If all you have is Reader, I don't think you can modify it. This goes for anyone on the web wanting to fill it out. EDIT: Of course I could be wrong. You best bet, if they want it as a web accesable document is to turn it into an html form. I totally agree with the "threes". It seems (seemed?) that way to me too. Only you forgot 3 hours. Yes, hours. When you mentally decide you are going to quit, 3 hours has pushed your body over the limit for its nicotine supply. It does get easier, really. At 6:30 a.m. tomorrow morning, it will be 4 months for me, smoke free. Quit (last drag) at 6:30 a.m. June 30th, 2003. Create an addiction (start smoking). Realize it's an addiction (Throw pack of smokes out of window to prove you can quit.) Realize it's a very strong addiction (pull into gas station 20 minutes after tossing pack out of window, to buy another pack). Attempt to quit cold turkey again (risk prison sentence for manslaughter). Attempt to quit the use of a drug by starting to take another drug. Cigs, the poor mans cocaine. Why do we call it that? Because when you get hooked, it's everyone else that ends up paying for your addiction. From the friends you lose by smoking, those you offend while trying to quit (and failing), and all those taxpayers paying your medical bills as you age. I really wouldn't be against a hospitals right to refuse service for smoking related diseases. (How's that for a can 'o worms?) Try this people, just freakin' quit. Better yet, don't start.
