That’s probably within the loading range of the canopies. So probably not a WL problem. how tight is the canopy in the Dbag ? If it is loose it could let the slider get off the slider stops. (Just throwing ideas)

How much are you loading the canopies? I had one high performance canopy who would often slam me if I went light. And super soft with an additional 4kg weight belt. not saying it is what happens to you, but could be some food for thought.

Actually I believe that it was indeed the rig paperwork. In France they have 4 different documents Harness, reserve, main and AAD. Plus the eventual works report, AAD service paper, insurance, license. Often the jumpers keep the paperwork in a plastic envelope. On the video, it looks like the whole booklet.

A pre-acid mesh era parachute probably deserves a display in a museum more than an AIR before potential use.

Is the result the same if the rig is worn properly? (Leg straps on and tight) is the result the same if packed by a different rigger ?

Maybe the passenger was not a paying passenger, but another experienced skydiver (even TI, why not) who was there to test the feasibility of Very High Altitude Tandem. Who knows. Yet again, I do believe that the crew involved in the jump are/were among the very knowledgeable in the high altitude jumping.
