Ok, after the fatality at Skydive Miami where the MLW adjustment point failed, sunpath issued a bullitin to inspect the anchor point for "Unusual or excessive" wear. Shouldnt this problem have been discovered on the last reserve repack? isnt that why we inspect the harness in the first place?
It has been said that a rigger in Miami was able to find a problem in other rigs AFTER the incident. I think this incident could have been avoided if a proper inspection of the gear had been done at the last repack. i mean this is not a hidden area of the harness.

Knowing it can fail, and where, can help to sharpen inspection skills. Sometimes the failure that has never happened is very hard to discern. Riggers get imaginative with failure scenarios, but they can't figure them all out. There have been adjustable harnesses for years and years without problems. Sometimes what we call complacency after an accdent is experience until something happens.
