Checking the condition of your connector links, whether metal or soft, should be a part of every pack job. If you have metal links a small dab of fingernail polish (pink is good ;)) on the barrel will let you know at a glance if the links are coming loose. I've never heard of a metal link failing if it was properly tightened (I could be wrong on that though); I think having both metal and soft would be kind of overkill.

Both slinks and metal links will fail if they are not put on correctly. The problem you describe can happen when the barrel loosens all the way and the lines load the link oddly. A slink that's not installed correctly (and I've seen them installed incorrectly) will fail just as readily.
