It was a male TM with a dutch woman (his wife if I remember correctly, also a skydiver) and her/their 10 or 12 year old son attached to the mom. " which is seen on the TM's Upper Right Connection,,,,,.. that looks just like the thing which could fail,,, in cold temps....or under a heavy loading,,,, at opening....... though I may be overreacting....
