I subscribe to the latter theory. (Within reason right? I mean you can do everything right and still die in ALL walks of life... crossing the street, eating dinner, taking a shower.) Now I tend to try and not argue with those that say they have a friend that "did everything right and still died." because I know it is a sensitive issue and some tend to view pointing out deceaced jumpers mistakes as speaking poorly of the dead.... however, Gus and a Twin Otter were in the same space at the same time... to me, this is an idication that not everything was "done right". I would not point that out except for the fact that the original poster is trying to get an idea or paint a picture. You could say "he/she did everything right" about a good number of injuries and fatalities in this sport and have someone turn around and show you where you are wrong. I just want to make sure that the original poster is aware that there is a "Generally, if you do everything right, you won't get hurt." school of thought among us too.
