trying to submit B license application to USPA without luck

I was signed off for my B about 2 weeks ago and was instructed to fax the completed form to USPA, just like I did last year with the yellow checkoff form but the fax number "has been disconnected."

No big deal there was an alternative way to send it in and an address printed below so I just enclosed a check for $20 and mailed it in.

Mail returned yesterday with a postal sticker "return to sender after several attempts."

I know the form is official USPA dated 2003. Was the fax # /submission address changed recently? Has anybody else been experiencing this?

The same thing happened to me. Whatever the address on bottom of the B license form was wrong. It was returned to me, and I think I got their correct address from myspace.

You probably have an old application form. The organization moved in 2006. Forwarding of phone and mail has probably expired.

no problems on this end.

faxed 3 b-lic applications had e-mail confirmation of lic-numbers within 24hrs.

used current forms, and checked website for current contact info and numbers.

dont forget to pull.

Thanks everybody. That's my answer. So much for mail forwarding from the USPO.
I'm very surprised the fax number had to be changed and even more surprised that it was not automatically forwarded or that there was not a forwarded message
