An old student of mine broke his femur when a pretty new Factory Diver fogged over. Something like 100 out of 300 people had their faceplate ice over completely at 26,000 - even though at least half of the helmets were brand new Oxygns. Anti-fog helps but does not guarantee lack of fogging/icing.

Alot had to do with the things I wrote in my post though. Helmets were left on for long periods of time before exit on the way to altitude and the Oxygen helmets with built in canulas and flip up face shields were mostly kept shut for warmth. A bad combination when exiting at sub zero temperatures. I was able to not have mine fog up during the record jumps because I was sitting right behind the pilot in Shark Air with NO heater, I never put my helmet on until the green light was given for climb out, and the pilot had his windows open all the way to exit altitude so the plane windows didn't fog up, so it was about -21 degrees all the way up. If the weather is cold out, such as winter jumping, flipping up the shield while under canopy is a good idea also. Be safe. In my limited time in the sky, it appears to have to be cold and humid (aka damp) for that to occur. Does that fit with your experience?
