Most of them are probably people who either forgot it was in the bag (stupid), or were too lazy to put it in their checked baggage and do the paperwork (more stupid). On the subject of 50 million prohibited items. Yes, all those bottles of water and shampoo, along with those nail clippers add up fast. I would expect nearly all of them to be in the first category (forgetful). Who would willingly take a chance on the TSA missing it, given the consequences? Lousy risk/reward ratio, and you'll (rightfully) lose your CCW as a result. I would love to know how many times CCW holders did take their weapon onboard but wisely didn't mention it once they discovered their fuckup. I bet it's a lot higher than 4600.
