Must not have looked very hard. For a 45 y.o. male in Phoenix, Healthnet has a 6k deductible, 0% coinsurance, $30 office visit policy for $167/month.

Humana has a 5.2k deductible, 0% coinsurance policy for $113/month.

Companion Life has a 2k deductible, 0% coinsurance for $248/month.
