Can't remember for sure, but I think it was pilot induced roll - error I guess. The coolest part was the thrust oscillation of the rocket motor just after startup. You can see the airframe shaking in the outside views and also see how much the pilot was getting shaken. I'm also not positive, but I think there was a problem with the on board computer during the first official attempt. It was supposed to provide proper pitch and roll information to the pilot kind of like a HUD with a bullseye. It failed so the pilot flew it manually.
