That means that either (a) the doctor has to put forth a lot more time/effort for the same note, or (b) the doctor put forth the same amount of time/effort and basically just cut-and-pastes the note in. You're proving my point with continuing to post about EMR in this thread, despite the fact that there's a WHOLE NEW THREAD about said topic. Patients often put stuff in the wrong area that if charting is done correctly then has to be cleaned up by someone that knows what he/she is doing.
