And bad management allows that to happen (they think saying "I only want to know that this is going to work." is firm leadership). So then you end up with crap like this. I'm sure some of the people making those reports were told that they had BETTER succeed, and they wanted to keep their jobs. Those of you who are managers, or who brief managers -- pay attention.
