The business district of most cities is usually pretty small - Denver in about 1 square mile, for instance - one small nuke could do a lot of damage if placed correctly. Oklahoma City bombing is a good model for such a terrorist act - imagine if it was a nuke. The good thing is that they are hard to make. The bad thing is that they are easy to lose. Also, with state sponsored terrorism, some countries can afford dedicate the money and resources to build one - if you have the time, money and access to natural uranium, you can easily build a cyclotron to generate enough U-235 to make a critical mass device like the Hiroshima bomb.
