How can an RSL fail? As long as it's connected it should work, right? Obviously I'm missing something here, can you explain it to me?

Easy...the lanyard can break, the clip can break, the ring on the riser could come off, or you can snag it on something and undo it. Nothing is infallible.

What he said... I've actually noticed my RSL open after I was back on the ground following a jump. So at some point I must have bumped the disconnet clip... when it happend I don't know... but it happened...

Reminds me of someone else's take on canopy flying - 'Be a pilot, not a passenger' - which is another nice way of putting it.
