It's a 4" by 4" piece of .090 aluminum with two slots and a hole. A type 8 belt goes through the slots and the hole accepts a 1/4" bolt that threads to the camera itself. I put some Ensolite on the bottom to angle it a bit and make it less slippery. I will post a picture if I remember.
