It almost did. This is a picture of a 15/100 inch crater left on the windshield of the Space Shuttle Challenger (STS-7 in 1983) when it was hit by a fleck of paint estimated to be 8/1000 of an inch traveling at a relative velocity of an estimated 3 miles per second.

And seeing as how it probably cost over two million dollars just to get that junk up there, the fastball is about as expensive as Barry Zito's.

Yep, they do. I don't know how they do it, but they manage.

The one sticking point in the story is that while the camera is expected to completely burn up on re-entry, the big ass tank won't, a few small pieces of it will make it all the way down. They're "hoping" those pieces will hit the ocean...

They can map and see all the junk floating in space, but can't guarantee where the shit hits the fan on earth... :S

The shuttle fleet is aging, and they are not building any new ones, and the current shuttles still in service are becoming too expensive and complicated to service.

NASA has elected to go with a totally new design and concept for future manned flight. It could mean a decade or so before the first flight of the new rocket. Which of course, probably means depending on Russia's Soyuz program to get our astronauts up and back down for the time being. :S
